Patient Case of Month of  July and August 2017
I have seven new cases, one patient from Dharamsala and six patient from Tibetan Sonamling Settlement Ladakh. At present all patients are going treatment at the different hospital in Delhi. I would like to thanks, all blood donors, doctors who and my friends who are always helping on my work.

Major Patient case of One year

Daily we are helping 1 to 2 patients and sometimes more than that, except weekend, as most of the government hospital is closed for OPD. Here we had listed patient with server medical problem.
Patient History: T J, 25 years male patients referred from Ladakh Hospital to AIIMS for further Evaluation on 25th May 2016.
Diagnosis: Congenital Heart Disease and Seizure.
Treatment: Case seen by Cardiologist and Neurologist AIIMS. Echo-cardiogram and MRI Brain Seizure Protocol report shows normal. The patient is on Anti Seizure medications and he is much better.

Patient History: – T R, 22 years male patient referred from Ladakh Hospital to AIIMS complain of right lower facial swelling in the month of June 2016.
Diagnosis: Malignant Tumor of the parotid.
Treatment: Right side total conservative parotidectomy done under general anesthesia on 13th June 2016.
Biopsy report: Pleomorphic adenoma. at present patient is doing well and continue doing his further study. review every 3 months at AIIMS Delhi.

Patient History: K P, 60 years the female patient from Jangthang (Ladakh) referred to Shri Ganga Ram Hospital for ERCP and further Evaluation. She under gone cholecystectomy with T- tube. The patient is having Severe Jaundice.
Diagnosis: Choledocholithiasis.
Treatment: On 13th Sep 2016, ERCP/EPT/Stone removal and Stenting has done. After 2 days removed 50 ml fluid from Perihepatic. On 20th Sep 2016 patient sent back to her place and much better. She is reviewing her treatment at Ladakh.

Patient History : D T, 70 Years male patient referred from Dharamsala Tibetan Delek Hospital to Shri Ganga Hospital Delhi in July 2016. Complain of Epigastric pain.
Diagnosis: Carcinoma of Stomach.
Treatment: Laparotomy with ileostomy done at SGGR Hospital. After eight days, patient discharge from the hospital. He underwent Chemotherapy eight cycles at Dharamsala and treatment guide by the Oncology SGGR Hospital.
At present all treatment finished and the patient is doing well. He’s reviewing doctor after every 3 months.

Patient History: T D, 30 years female patient referred from Ladakh Hospital to AIIMS, complete perineal tear after delivery.
Diagnosis: Perineal Tear.
Treatment: CPT Repair was done on 8th Oct 2016.Got discharge on 11th Oct 2016. Every 6 months she is visiting AIIMS Gynaecology for follow up.

Patient History: T C,
30 years pregnant lady with severe anemia, jaundice and Vitamin B12 deficiency referred from Ladakh. First shown to AIIMS and then to Safdarjung Hospital in the 23rd Oct 2016 under Gyanae and Hematology Department.
She got admitted two times during her pregnancy to rule out her disease. On 23rd Jan 2017. She delivered a baby boy after one day baby developed severe Jaundice. After phototherapy and blood exchange for the baby. 27th Jan 2017 got discharged from Hospital at present both mother and baby are fine and back to their place.

Patient History: S Y, 14 years female student of TV Ladakh referred from Ladakh complain of left flank pain and history of Lt URS and DJ Stending. On 20 Jan 2017 shown to AIIMS Urology for further evaluation.
Investigation: RDS and IVP for both kidney done.
Treatment: Patient is better, follow up every 6 months at AIIMS.

Patient History: T C, 8 months baby complain of severe lower chest infection and cardiac problems.Referred to AIIMS for further evaluation shown to AIIMS Pediatric Cardiology on 8 March 2017.
Diagnosis: PDA with lower respiratory tract infection.
Treatment: Echo report shows normal, his chest infection rule out and treatment started by AIIMS Pediatric Doctor.

Patient History: R L, 46 years female, T.C.V Ladakh staff, referred from Ladakh Hospital to AIIMS Delhi for evaluation of Fibroadenoma of right side breast, show to AIIMS Breast Cane clinic.
Diagnosis:?Ca Breast.
Treatment: Gun Biopsy done, report shows normal, At present patient is doing well and show is follow up every six months at AIIMS.

Patient History : D D, 34 years female patient referred from Ladakh Hospital case of collection of fluid and small mass in uterus, on December 2016, shown to AIIMS Gynecology.
Diagnosis : ? Tuberculosis
Treatment : Removed fluid from uterus and on Tuberculosis medication, Follow up every 6 months at AIIMS
